Social Services
Sundale's Social Service Department provides services to meet the psychosocial, emotional, social and cultural needs of our residents and families.
Social Services is available to serve as a liaison between residents and community resources and can assist with community referrals and counseling referrals.
Social Services is also available to assist
with the following services:
- arranging for adaptive equipment, clothing and personal items
- contact with families regarding changes in health, goals, discharge planning, and encouraging participation in patient care planning
- making referrals and obtaining services from outside entities
- assisting residents with financial and legal matters
- discharge planning
- arranging provisions for needed counseling
- building relationships between residents and staff and teaching staff how to understand and support resident's individual needs
- promoting actions by staff that maintain and enhance residents dignity and individuality
- assisting residents in determining how they would like to make decisions regarding health care and whether they want someone else involved
- assisting staff to inform residents and families about their health status and health care choices and ramifications
- finding options which meet the physical and emotional needs of each resident
- providing alternatives to restraints and drug therapies
- meeting the needs of residents who are grieving
